These support plans are regularly reviewed and updated to ensure they remain responsive to the person’s changing needs and circumstances.
Multi-Disciplinary Approach
Supporting people with complex needs often requires a multi-disciplinary approach involving professionals from various disciplines, such as occupational therapists, behaviour specialists, and healthcare professionals. We partner with several housing associations and property owners to find suitable and affordable places for people to live, ensuring that the chosen accommodation meets the person’s unique requirements. Our dedicated support team works closely with each person, their families, and other relevant stakeholders to develop a transition plan that outlines the steps involved in the move to supported living. A very good experience from booking the carer through to the time the carer spent with my mother. I would use this company again if further care was required. Always punctual and offering friendly, sensitive help with washing and dressing each morning. To have the same carer every day for a month was wonderful and the alternative carer, when on rota, was also kind, friendly and effective.
